Abstract

Objective: To compare treatment strategies and outcomes between high risk (HR) and very high-risk (VHR) patients initially treated with radical prostatectomy. Methods: This was a retrospective study of patients with HR and VHR prostate cancer patients initially treated with radical prostatectomy. We analyzed demographic and clinical variables. We performed Kaplan- Meier test to compare oncological outcomes. Results: There were a total of 112 patients (57 HR and 55 VHR patients). Median follow-up was 93 months. Postoperative prostate specific antigen was undetectable in 76.8% of HR and 51.8% of VHR patients. Moreover, VHR group received postoperative radiotherapy in 72.7% vs. 51.8% in the HR group (p = 0.03). Biochemical persistence was higher for VHR group (39.3% vs 19.6%; p = 0.03). Conclusions: We found a higher rate of postoperative RT and biochemical persistence in VHR group. These results are useful to counseling VHR patients for better estimating prognosis and decision making.
